A strategy to help individuals manage their health better
Humanim has become a licensed Health Home provider for the State of Maryland. The Health Homes will operate under the Psychiatric Rehabilitation Program. Health Homes will assist individuals with serious and persistent mental illness in improving their overall wellness. It is a strategy to help individuals manage their health better and live in the community longer. We will take a person centered approach to addressing the individuals behavioral, somatic, and social needs. Ultimately, we hope the program reduces the utilization of emergency services and preventable hospitalizations, and increases the life expectancy and overall wellness of our clients.
