This month we spotlight our Health Homes program, which addresses healthcare and lifespan disparities for those who experience mental health challenges or disabilities. Health Homes is a wraparound program for our Human Services clients, designed to increase client health, wellness, and life expectancy through health education, intensive clinical coordination, and medical case management.
Our Health Homes team empowers clients with knowledge about their physical health, while ensuring they utilize the health and wellness resources that are available to them.
Last year Health Homes partnered with Speak(easy) Howard to provide continuing end-of-life care workshops and seminars, as well as launched their first Annual Consumer Health Fair.
